Bilingual (Spanish/English) Intensive Clinical Case Manager for HIV Positive Young Adults

This position is based in a facility licensed by the Community Care Licensing (CCL) Division
of the Department of Social Services in the state of CA. All employees of this facility must clear
a livescan background check. Any person with previous convictions at any point in their lives,
will not receive a clearance from CCL. This includes all convictions: felonies, misdemeanors and
DUIs.

Previous convictions are NOT an automatic bar to employment at other non licensed Larkin Street
Youth Services sites. Each case will be considered on its own merit.

Larkin Street’s continuum provides a range of housing options—from immediate emergency
shelter to permanent supportive housing—in addition to essential wraparound services that offer
kids the resources and skills they need to exit street life. Supportive services include education,
technology and employment training; healthcare, including mental health, substance abuse, and
HIV services; and case management.

Assisted Care/After Care (AC/AC) Housing Programs

Assisted Care is a 12 bed RCF-CI (licensed) facility for youth (18-24) living with HIV/AIDS.
After Care provides housing subsidies, case management, and support services to youth (18-24)
housed in the community who are living with HIV/AIDS. The program offers comprehensive
psycho-social, educational, vocational, and employment opportunities to the participants served
to ensure their successful transition into greater independence and self sufficiency. There is an
emphasis on serving young adults with behavioral health and substance abuse needs.

PRIMARY RESPONSIBILITIES

The primary role of the Intensive Clinical Case Manager is to provide clinical casework and
crisis intervention services to high-risk homeless and/or marginally housed, HIV positive,
transitional aged youth who present with significant behavioral health needs. The Clinical Case
Manager is responsible for intake and assessment; case planning and contracting; individual,
family and group counseling; resource brokerage and referral; case coordination and advocacy;
follow up and after care and recordkeeping. The Clinical Case Manager works closely with other
programs of Larkin Street Youth Services, community resources and the program’s residential
counselors to insure coordinated and effective services.

QUALIFICATIONS
• Master’s Degree in Counseling, Psychology or Social Work required
Bi-lingual Spanish/English required
Licensed or license eligible required
Ability to work Saturdays and some weekday evenings
2-3 years of experience working with high-risk youth in a residential or outpatient mental
health setting.
Ability to identify, assess and intervene effectively with mental health symptoms and
behaviors
Ability to notice symptoms of use and abuse; working knowledge of recovery and
treatment philosophies.
Ability to provide HIV education and prevention, as well as substance abuse counseling.
Working knowledge of issues facing homeless and LGBTQ youth.
Must be a self-starter with excellent follow-through skills.
Must also have ability to work with diverse staff, clients, and volunteers.
Proficient in Microsoft Office (Word, Excel, and Outlook)
Ability to multi-task and efficiently manage priority action items

Salary range: $38,000-$44,000 DOE
