Job: Case Manager II - Bilingual: Hamilton Family Center

POSITION DESCRIPTION

Position Title: Case Manager II - Bilingual
Program/Dept: Transitional Housing Program (THP)
Reports to: Case Management Coordinator
Classification: Regular, Full Time, Non-Exempt position
Salary Range: $17/hr + $0.50/hr Bilingual Premium / Full Benefits Package

The mission of Hamilton Family Center is to break the cycle of homelessness and poverty. Through a Housing First approach, we provide a continuum of housing solutions and comprehensive services that promote self-sufficiency for families and individuals, and foster the potential of children and youth.
Hamilton Family Center services include HFREC; the largest emergency shelter and THP the largest transitional housing program for families in San Francisco; the Dudley Apartments, a permanent supportive housing site for families and individuals; First Avenues, our innovative rapid re-housing and homeless prevention program; Project Potential, our services for children and youth; and Myhousing.org, our affordable housing, homeless prevention and community banking website.

The Transitional Housing Program is a twelve to eighteen-month housing program providing 20 families with housing and supportive services designed to enable them to return to stable, independent housing and economic self-sufficiency. The program provides case management, employment readiness activities, job retention support, intensive housing search assistance, life skills workshops, and referrals for ancillary services as appropriate. The Bilingual Case Manager II will provide families, including those who are monolingual Spanish speaking, with case management and support to assist them with gaining stability and obtaining permanent housing.

Primary Duties and Responsibilities (other duties may be assigned):
• Participate in family intake & orientation to the program;
• Provide residents with case management services, including but not limited to: one–to-one and group counseling, workshops, advocacy, mental health, physical health, domestic violence, substance abuse recovery support services, family reunification and child welfare advocacy & life skills trainings.
• Identify, network with and refer residents to other community-based services as appropriate.
• Develop, implement and document family action plans with the residents that identify and address their needs.
• Learn, understand, adhere to, & reinforce with residents all program policies, rules and expectations for participation in the program.
• Provide support services & information that is consistent with the program’s stated goals & objectives;
• Collaborate with program staff, community partners, & service providers to support families; Interface with other program staff to assure communication, consistency & teamwork.
• Observe client confidentiality & HIPAA protocols; Maintain program’s standard of documentation of case management services; keep accurate records, files, correspondence & statistics in both written form and via data entry. Prepare reports & presentations of information as required.
• Attend and participate in weekly case review meetings and present progress reports on families; attend regular staff meetings and represent Hamilton in the community as needed.
• Comply with program and citywide policies & procedures; attend hearings and arbitrations as part of the shelter grievance procedure.
• Support and create active participation in self-governance by the participants in program services.
• Translate English to Spanish programmatic written materials as needed and provide verbal translation support, including translation for community meetings, workshops and events.
• Follow organizational established procedures & policies pertaining to communication, health/safety standards, & emergency response procedures.
• Maintain & promote an atmosphere of dignity & respect in line with the philosophy & policies of Hamilton Family Center; Maintain & promote the cooperative, harmonious, teamwork environment HFC strives to promote within the workplace;
• Other duties as assigned

Qualifications, Skills and Abilities:
• Bilingual English/Spanish as demonstrated through agency proficiency exam.
• Bachelors degree in Social work, Psychology or related field from an accredited college or university; or a minimum of 5 years of experience performing case management duties in a health or human services field.
• One year of professional experience working in a residential facility providing support services
• Two years experience working with homeless families.
• Must be available for occasional evening hours as work schedule includes some evenings.
• Experience working with mental health related issues, substance abuse, domestic violence, HIV/AIDS related issues, Child Protective Service agencies, HIPAA and crisis intervention techniques;
• Excellent written and verbal communication skills; computer literate – ability to operate IBM compatible computers including regular entries into a web-based client database and demonstrated ability to use MS programs such as Word, Excel, Outlook, PowerPoint, etc.
• Essential functions of the position may require use of a telephone, work at a standard computer terminal and the ability to follow reasonable ergonomic recommendations, sit for long periods, stand, reach, bend, lift and carry up to 40 pounds.
• Position requires routine TB (Tuberculosis) testing and documentation (post-offer).
• Background checks required (pre-offer).
• Valid CADL and clean DMV record strongly preferred; Able and willing to travel locally as needed.
